Any student athlete seen by a physician must return to the athletic training room for follow-up and final clearance prior to returning to any sport.  It is required that a return to play note be obtained by any student athlete who sustains an injury or contracts an illness in which they need to see a physician.  The return to play note as well as any documentation from the physician visit that explains the type of injury or illness and any restrictions or special care must be turned in to the Athletic Trainer prior to returning to any sport. This not only helps to ensure the safety of our athletes, but is also a state requirement.  Certified Athletic Trainers work under the license of a physician and must follow their instructions - whether verbal, written, or established.

If you see a physician for an injury or illness, bring back a note!
